YoloCares was the first established independent hospice in Sacramento County, and continues today to serve those with life-limiting illnesses.
YoloCares is a member of the California Hospice Network, working to provide quality end-of-life care for individuals and support to their loved ones. YoloCares carries the philosophy of honoring the whole person—medically, spiritually and psychosocially. This is accomplished through four programs.
Palliative Care: This highly specialized and coordinated care improves the quality of life for patients who are working with physicians when there is potential for illness to be cured or better managed. YoloCares collaborates with doctors on a treatment plan that supports patients and gives them the best quality of life possible. This care can be administered either at home or in a hospital setting.
Hospice: YoloCares helps terminally ill patients set goals for their comfort and end-of-life plans. Everyone deserves to die with dignity, and even when the end is near, there is still life to live. Working closely with both patients and their loved ones helps improve those final days, months, or years.
Adult Day Program: Galileo Place—a 6,000-square-foot facility with top-of-the-line amenities—is home to YoloCare's adult day program for those who need assistance and companionship while their caretakers are unable to be present.
Center for Loss & Hope: YoloCares offers a wide array of grief programs for both children and adults. These therapeutic opportunities are free of charge and aim to bring healing and closure following the loss of a loved one.

The Sacramento County Board of Sipervisors approved, at its Oct. 22nd meeting, up to $1,137,788 in funding to complete construction on Joshua House, a facility that provides end-of-life comfort care for homeless individuals with terminal illnesses, after being discharged from an acute care hospital.

Galileo Place, YoloCares’ new Adult Day Program, was awarded $225,000 from the Yolo County Board of Supervisors, which will provide scholarships to low-income families.
